What are copays, coinsurance, and deductibles? How do they work in my insurance plan?
Copays, coinsurance, and deductibles are different ways that you share the cost of your healthcare with your insurance provider. Here's a breakdown:
-
Copay: A fixed amount you pay for a covered healthcare service. For example, you pay $20 for a visit to your primary care physician, and your insurance will cover the rest.
-
Coinsurance: This is the percentage of costs you pay for a covered service after you’ve met your deductible. For example, if your coinsurance is 20%, and the total cost of a procedure is $500, you would pay $100, and your insurance would cover the remaining $400.
-
Deductible: The amount you must pay for healthcare services before your insurance begins to pay. For instance, if you have a $1,000 deductible, you’ll pay the first $1,000 of your healthcare expenses out-of-pocket, after which your insurance will start covering a more significant portion of your costs.
Why do I need to provide my credit card information if I already have insurance coverage?
Even though you have insurance coverage, there may be instances where you still need to pay out-of-pocket for certain services. This could include copays, coinsurance, insurance lapse, or cancellation fees that go beyond your insurance coverage limits. Your credit card information is used to securely cover these expenses at the time of service or for billing purposes, ensuring that any remaining balances are paid promptly.

What should I do if my insurance changes or expires?
If your insurance changes or expires, please email us at info@likeaphoenix.net with the front and back of your updated insurance card. We will update your file accordingly to ensure everything is up to date.

Will I be charged directly once my appointment is completed?
We typically do not charge any copays or deductibles at the time of your appointment. Charges are processed only after we receive the Explanation of Benefits (EOB) from your insurance provider. If the EOB indicates there is no copay, we will adjust your billing accordingly. You will receive the same EOB that we receive from your insurance provider. Rest assured, we strictly follow your insurance plan's guidelines and will not charge you beyond what your insurance allows. We are committed to full compliance with your insurance coverage.
If I have MassHealth, do I have to fill out the credit card information form?
Yes, you do. If there is a lapse in your insurance that you didn’t inform us about, you will be responsible for the session cost.

Can I choose which one I use if I have two or more insurances?
No. The insurance companies decide which one is primary and which is secondary.
Do I need to let you know if I have a health savings account (HSA)?
Yes. If we do not know about your HSA on time, we will charge you for any potential copays and deductible fees.
Do I have to give you my sex?
Yes. To ensure that insurance covers your sessions, we are required to submit the sex listed on your insurance policy. Please provide the sex that matches what your insurance has on file to avoid any billing issues.
Do I need to tell you when my insurance changes?
Yes. Please inform us immediately of any changes to your insurance to prevent being billed the full session cost out of pocket.
Do I need to let you know if I have a flexible spending account?
Yes. We will charge you for any potential copays and deductible fees if we do not know that you have a flexible spending account on time.
